Through a right parieto-occipital craniotomy, the dura has been opened and cortex is exposed. The initial trans-sulcal approach is made in the superior parietal lobule with the operating microscope.

The right lateral ventricle has been reached and the meningioma is exposed; an initial biopsy is taken.

An ultrasonic aspirator is used to internally debulk the meningioma.

The meningioma is further mobilized from the ventricular walls and removed in piece-meal fashion.

After the majority of the meningioma has been removed, the endoscope is used to explore the ventricle. A small tumor remanant is seen and removed. A second very small tumor residual densley attached to a large vein is cauterized and left.

The ventricle is flooded with irrigation; no bleeding is seen.

The retractors are removed and the dura is then closed.
