A midline dural and arachnoid opening is performed exposing the cerebellar tonsils and large tumor mass extending through foramen of Magendie.

The tumor is densely adherent to the lower brainstem and upper cervical spinal cord.

The inferior portion of tumor is removed with sharp dissection exposing the dorsal surface of the upper cervical spinal cord.

The large tumor mass in the 4th ventricle is now removed with the ultrasonic aspirator and micro-dissection.

The most superior portion of tumor is now mobilized and removed from the 4th ventricle.

A densely calcified thin remnant of tumor is left attached to the floor of the 4th ventricle.

Endoscopic inspection of the 4th ventricle showing a near complete tumor removal except the calcified portion adherent to the ventricular floor.
